Cancellation Policy and No Show.
We see the therapeutic relationship as the most important aspect of treatment. If a client has multiple late cancellations or no shows, the provider will discuss the impact, the barriers, and the purpose of treatment. In fairness to both our clinical providers and other clients, the provider kindly asks the client to cancel and/or reschedule the appointment no less than two business days in advance. For example, if the client has an appointment Tuesday at 4:00 PM, appropriate notice would be to cancel the appointment by Friday at 4:00 PM. If the client has an appointment Monday at 9:00 AM, appropriate notice would be to cancel the appointment by Thursday at
9:00 AM. Should the appointment be canceled without sufficient notice or the client does not attend the appointment, the client will be required to pay for the full cost of the appointment, including but not limited to, the copay, the deductible, and the amount for which the insurance would have reimbursed Dr. Hoffman for the appointment had it occurred.
Late Arrival:
If the client is late for the appointment or does not show, please be aware that the client will lose that amount of time. The appointment will still need to end at the scheduled time, even if the client is late to the appointment, out of respect for the time of other scheduled clients. If the client is more than 15 minutes late, the client’s appointment may be forfeited.
Appointment Reminders:
Hoffman Psychiatric Services sends email, text and/or phone reminders to clients as a courtesy. It remains your responsibility to keep your appointment whether or not you receive reminders from us.
Late Appointments:
If you are late for your appointment, please be aware that you will lose that amount of time. Your appointment will still need to end at the scheduled time, even if you are late to your appointment, out of respect for the time of other scheduled clients. If you are more than 15 minutes late, your appointment may be forfeited, and a late cancellation fee will be charged.
Ongoing Cancellations and No-Shows:
Regular attendance at scheduled appointments is a key component of successful counseling. Please note you will forfeit your ongoing appointment time slot if you cancel or no-show more than three times. Your provider may reevaluate your needs, desires, and motivations for treatment at that time. Hoffman Psychiatric Services reserves the right to terminate the clinical relationship if more than three appointments are missed without proper notification.
Lapses in Treatment:
If you have a weekly appointment with Dr. Hoffman, and do not attend an appointment for four consecutive weeks, you may be discharged and will no longer be considered an “active” client ofHoffman Psychiatric Services. You can be placed back on her waitlist, but for safety and liability purposes, you cannot be left active for longer than four weeks, unless you and Dr. Hoffman have agreed in advance to a different meeting schedule.
